Purpose

The Children's Orchestra Society (COS) documents student activities through photographs, videos, and audio recordings, capturing moments during rehearsals, performances, classes, and dedicated events. These materials are intended to support COS's mission of promoting music education and may be used for educational, promotional, and archival purposes.

Consent Options

I GIVE CONSENT. I hereby grant permission for the Children's Orchestra Society to capture photographs, film, record, and/or otherwise document my child's image, likeness, name, voice, and musical performance. I acknowledge and agree with the following terms: - These recordings may be used, edited, altered, reproduced, published, or given by COS across various media platforms (including print, broadcast, digital, and social media) for publicity, educational, or promotional purposes. - Such usage may occur without further notification, review, or compensation to me or my child. - All photographs, recordings, and related materials will remain the exclusive property of the Children's Orchestra Society. COS reserves the right to edit, adapt, or integrate these materials with other content as deemed proper.

I DO NOT GIVE CONSENT.  I do not grant permission for my child to be photographed, recorded, or included in any public media. COS will make reasonable efforts to exclude my child from such materials; however, I understand that incidental appearances in group or background footage may occur.

Acknowledgment and Agreement

I understand that participation in COS activities is not contingent upon providing consent for media use.

I hereby acknowledge that any formal disputes arising from this agreement shall be settled through binding arbitration, by the rules of confidentiality, in the State of New York.
